Salary: $50,000

Primary Objective:

Best Point is seeking an individual that delivers an array of services which meet the individualized mental health needs of adult caregivers. Performs a variety of functions in multiple settings, (including office, telehealth, home, and community) displaying an understanding of family dynamics, cultural preferences, and community environments. Delivers services to client and/or any other individual who will assist in the client’s mental health treatment.

Primary Responsibilities:

Direct Client Service

  • Conducts agency based as well as community-based and home-based individual, group, and family therapy and/or CPST.
  • Provides a comprehensive and complete diagnostic assessment of each client referred for mental health service, which includes a conceptual and clinically significant behavioral or psychological picture.
  • Develops treatment plans, formulates, and facilitates intervention objectives to meet client and family’s needs.
  • Evaluates progress of each client, while providing coordination and/or assistance in crisis management and stabilization as needed.
  • Make specific efforts to incorporate an awareness of culturally diverse populations through an understanding of the nuances of an individual and family’s cultural frame of reference.

Administration

  • Completes all required documentation including but not limited to DAFs, service tickets, progress notes, quarterly outcomes assessments, service plans, discharge summaries, and other reports as required, while meeting or exceeding all monthly and quarterly department productivity (UR) requirements.

Internal and External Collaboration

  • Consults and collaborates with internal as well as external resources on behalf of the client and family.
  • Consults and collaborates with internal as well as external agencies, organizations, and entities on behalf of the department.
  • Develops and maintains a working knowledge of community resources.
  • Maintains updated information regarding human services, health, mental health, legal, and educational/vocational systems of care.
  • Attends agency and departmental meetings. Serves on agency wide committees.

Quality Assurance and Effectiveness

  • Delivers interventions based on best practice models.
  • Attends agency trainings or professional seminars/conferences as required or necessary in order to maintain compliance with professional licensure and to increase professional knowledge and skill base.

Fiscal Responsibility

  • Meets or exceeds monthly and year to date department productivity requirements (UR).

Minimum Education, Experience and Other Skill Requirements:

This position requires a Master’s degree in Social Work or related field from an accredited college or university plus a minimum of one year of clinical experience in working with children and families. Certification (PC, LSW, MFT, PCC, LMFT or LISW) required. LISW, PCC or IMFT strongly preferred. Must possess minimum technical skills including familiarity with Microsoft Windows and ability to use standard Microsoft Office applications.

Work Environment:

Exposed to office, community, and home environments. Exposed to adults with mental health diagnoses and associated children. May be exposed to shouting and clients who may be verbally and physically aggressive.

While performing the duties of this job, the employee is regularly required to: Regularly required to sit, stand, walk, bend, and lift up to 50 pounds. Ability to operate a motor vehicle safely in changing weather conditions required.
