The Community Service Agency (CSA) uses a wraparound model to support youth (ages 3-20) and their families to be successful in the home or community setting. CSA uses a family-centered approach and our services are culturally sensitive. Our Intensive Care Coordinators (ICCs) support youth and families by building new skills, helping them to access community support, and ensuring that all service providers are working together. ICCs collaborate with various community systems including: schools, Primary Care Providers, Department of Children and Families, courts, and other mental health agencies.
Additional opportunities for ICCs include: training in trauma informed care models, training and utilization of the evidence based Wraparound model, potential quarterly bonuses and productivity incentives and participation in on-call phone support for a monetary stipend.
The ideal candidate is a team player that is organized, compassionate, and motivated to help youth and their families meet their goals and improve their lives.
Responsibilities:
  • Facilitate the development of a Care Planning Team (CPT) which will include both formal and natural supports.
  • Coordinate and communicate with members of the CPT to ensure implementation of the Individual Care Plan (ICP) and Safety Plan.
  • Meet with youth and family regularly. Visits may occur in the office or in a community location such as the family's home, school, etc.
  • Monitor and review progress towards Individual Care Plans (ICPs).
  • Complete documentation to reflect services provided in a timely manner.
  • Carry a caseload as assigned.
  • Communicate with Managed Care Entities to ensure authorization for services provided.
  • Meet productivity standards for hours worked.
  • Build and adjust schedules as necessary to meet the needs of persons served. Afternoon/evening hours are required. Staff must be willing to schedule and hold appointments based upon the needs of the families served.
Qualifications:
  • Master's Degree in a Mental Health or a related field preferred.
  • Bachelor's degree in a human services field and one year of relevant experience working with families or youth.
  • Valid driver's license, good driving record, and reliable vehicle. Willingness to travel and transport youth as needed.
Schedule: Full time, M-F 40 hours per week
