Job Summary:
We are looking for a dedicated and empathetic Case Manager to join our team. As a Case Manager, you will be responsible for providing support and guidance to individuals in need, helping them navigate through challenging situations and access necessary resources.

Responsibilities:
- Conduct assessments to determine clients' needs and develop individualized care plans
- Coordinate services and referrals to assist clients in accessing healthcare, housing, employment, and other support programs
- Provide crisis intervention and emotional support to clients facing difficult circumstances
- Monitor client progress and adjust care plans as needed
- Collaborate with healthcare providers, social workers, and community agencies to ensure comprehensive care for clients
- Advocate for clients' rights and empower them to make informed decisions about their well-being
- Maintain accurate and up-to-date case notes and documentation

Qualifications:
- Proven experience in supervising, crisis intervention, mentoring, motivational interviewing
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced environment with diverse populations
- Knowledge of community resources and services
