**DESCRIPTION**
Duties are performed under the supervision of the Program Administrator for Adaptive Recreation Center and GR Services.
Ability to work flexible hours Monday-Friday with the possibility of working some evenings and weekends 40 hours a week.
This position is responsible for working closely with Service Coordination and Provider Services to develop, coordinate and monitor outcomes and strategies for individuals in IDD GR Services
This position requires the ability to: work closely and positively with the local authority, family members, advocates, contractors, etc.
Conducts self in a positive and professional manner; completes quality documentation; excellent verbal and written communication skills
Monitors the implementation plan outcomes on an on-going basis
Exercise initiative and independent judgment
Meets contract requirements regarding time-lines and processes.
Capable of working flexible hours to provide Community Support Services to General Revenue individuals who live with their natural family or their own home and ISS day services as needed.
Responsible for assisting individuals to access community services, providing transportation, assisting with grocery shopping and household chores, medication monitoring, habilitation training, and other duties as required by the services requested by the individuals Service Planning Team and their Person Directed Plan.
Transportation of consumers may include using your personal vehicle.
Must meet established performance targets.
**QUALIFICATIONS** A high school diploma or equivalent
Experience working with individuals with intellectual and developmental disabilities.
Position requires knowledge and skill in the use of a computer and common office software
Must have a current Texas Driver’s License and qualify for Center insurance coverage.
Must be willing to transport clients and receive mileage reimbursement.
