Beverly Farm Foundation is Hiring a Community Day Service Coordinator/QIDP Near Godfrey, IL

Responsible for supervising active treatment at all times when assigned individuals are in the D.T. program. Develop program areas and plan services to provide for individuals’ needs and include training needed for inclusion in the community as appropriate.

JOB DUTIES:*
A. Care of Individuals Receiving Services:

  • Provide a clean and safe training environment for all individuals. Follow all safety and health practices established by the Agency.
  • Be an advocate for the individual. Decisions are made to coincide with individual needs and rights.
  • Do daily check of program area to ensure cleanliness, maintenance and safety of the building and proper operation of equipment. Communicate problems to appropriate personnel.
  • Assist individuals as defined by their IHP. Assist in all activities of daily living, which includes dressing, personal hygiene, bathing, toileting, mealtime activities and behavioral interventions as needed.
  • Approve supply requisitions to meet individuals identified needs and ensure equipment for training program is available.
  • Report all cases of individual abuse or neglect on the part of the individual, staff or visitors according to policy.
  • Assist the Nurse as needed and permitted by law.
  • Ensure all health, physical, mental and behavioral changes/concerns in individuals receiving services are communicated to appropriate residential personnel.

B. Active Treatment:

  • Supervise active treatment by ensuring that Lead Trainer and Trainers follow Individual Habilitation Plans. Ensure active treatment is provided as determined by the Interdisciplinary Team.
  • Perform duties consistent with improving the quality of habilitative activities and increasing the vocational training opportunities for the individuals receiving services. Train staff in providing functional, outcome based habilitative activities and vocational training opportunities.
  • Demonstrate to assigned staff what is expected and give feedback to them regarding their performance. Model the most effective habilitative and vocational training for individuals receiving services.
  • Assist in ensuring that individuals’ doctor and dental appointments during D.T. hours are met.
  • Provide a variety of innovative, age appropriate functional activities based on each individual’s identified needs.

C. Supervision:

  • Schedule assigned staff to ensure adequate supervision of active treatment and program management and keep an attendance record of assigned employees.
  • Coordinate transportation between program areas and residential programs as necessary.
  • Evaluate assigned employees performance as scheduled in personnel policies. Oversee counseling and disciplinary actions as necessary.
  • Ensure staff are using the computer to clock in/out and transfer appropriately. Report any concerns to appropriate personnel.
  • Approve holidays, personal days and early work releases for employees assigned. Arrange for program coverage when Lead Trainer or Trainers call off or do not report to work, including coverage for bus monitors.
  • Conduct assessments of staff training needs. Implement staff training or appropriate support strategies to remediate, inadequate performance. Engage in activities consistent with increasing staff training and morale.
  • Coordinate Program Team meetings for training area at least monthly, complete an inservice sheet and turn in sheet to D.T. Front Office.
  • Ensure staff receive training on all Agency health and safety Policies and Procedures. Complete mandatory inservices and attendance sheets as needed according to yearly inservice schedule.

D. Interdisciplinary Team Meetings:

  • Prepare and coordinate required assessments and information for all Individual Habilitation Plan meetings for assigned caseload by following policy guidelines and target dates.
  • Implement programs from the IHP meetings for caseload by following policy guidelines and target dates.
  • Attend all IHP meetings for individuals when assigned as a member of the Interdisciplinary Team. Plan for incidences when unable to attend IDT meetings by ensuring an appropriate substitute will attend.
  • Conduct, develop and implement IHPs for individuals as assigned.

E. Documentation:

  • Ensure safety drill reports are completed correctly and turned in to the D.T. Office within 24 hours after drill is held.
  • Audit assigned individual charts and purge records on a semi-annual basis.
  • Maintain permanent records of each individual's objectives. Update permanent record for each annual staffing.
  • Ensure all information is current in individuals’ files for assigned caseload.
  • Write measurable monthly progress notes including appropriate comments and changes when needed for assigned caseload within 2 days of due date. Review, sign and make appropriate comments and coordinate changes with the residential QMRP as needed. Ensure needed criteria changes and/or comments from QMRP are received within a week after monthly progress notes are completed.
  • Update Individual Activity Schedules within 24 hours of implementation of IHP.
  • Review individual ID note entries and include documentation of visits from the individual’s QIDP.
  • Observe all assigned individuals in the D.T. program and enter the observation in the individual's record as needed, but at least monthly.

F. Interpersonal Skills & Work Habits:

  • Display a positive attitude, which exemplifies the philosophy that individuals receiving services deserve respect and care with dignity.
  • Communicate and interact with persons receiving services in a positive manner.
  • Work well with others as a Team member.
  • Express self effectively and appropriately to others in both verbal and written communication.
  • Report to work as assigned by Supervisor and in a timely manner with limited absences or tardiness.
  • Display respect toward supervisor, peers and subordinates.

G. Production Activities:

  • Ensure the accuracy and timeliness of payroll for individuals receiving services and turn in individual payroll to the Contract Procurement office per established policy.
  • Conduct and monitor time studies on contracts according to Department of Labor standards. Monitor compliance to all Department of Labor standards. Ensure accuracy and timeliness of time studies.
  • Manage ongoing vocational work activities and ensure the quality of work completed.
  • Coordinate and implement any training of staff or workers that is necessary to meet the requirements of all contractors.

H. Perform Other Duties as Assigned:

  • Other expectations may be outlined for specific training areas.
  • Completion of additional tasks to meet the needs of the individuals receiving services may be required.

Equivalent combination of education and/or experience as follows:

  • Undergraduate degree in education or human service field.
  • One or more years experience working with individuals with developmental disabilities.
  • Ability to attain status as a QIDP if not already qualified
  • Ability to be a contributing team member and work cooperatively with staff and agency personnel, families, and community.
  • Good writing and communication skills required.
  • Sufficient physical stamina and mobility for lifting, carrying, pushing and pulling.
  • Ability to stay calm and not become angry when working with individuals receiving services who are upset.
  • Eligible for insurance by possessing a good driving record.
  • Ability to supervise staff and individuals receiving services effectively.
