Beverly Farm Foundation is Hiring a Community Day Service Direct Support Person Near Godfrey, IL

JOB SUMMARY: Assist the Supervisor in meeting individuals’ needs as specified by the Habilitation Plan. Implement all services specified in the Habilitation Plan for individuals assigned.
Job Duties
  • Care of Individuals Receiving Services:
  • Provide a clean and safe training environment for all individuals.
  • Review, follow and understand all safety, health and emergency policies and procedures established by the Agency.
  • Be an advocate for individuals receiving services. Make decisions to coincide with individual needs and rights.
  • Assist individuals as defined by their IHP. Assist in all activities of daily living, which include dressing, personal hygiene, bathing, toileting, mealtime activities and behavioral intervention as needed.
  • Report all cases of abuse or neglect to persons receiving services on the part of the individuals, staff or visitors according to policy.
  • Assist in the safe transportation of individuals to, from and between Development Training programs (i.e.: See Bus Monitor Job Description for more detailed expectations).
  • Check individuals as they leave the program area for a clean, neat and appropriate appearance.
  • Set up training activities before individuals arrive at the program. Put away training materials in appropriate place immediately after use.
  • Inform Supervisor of all health, physical, or behavioral changes in individual receiving services. Inform Supervisor of any circumstance affecting individuals’ emotional or physical well being.
  • Carry out individual Occupational Therapy, and Physical Therapy programs as assigned.
  • Assist Nurse as assigned and as permitted by law.
  • Active Treatment:
  • Provide training, conduct activities, and perform duties consistent with meeting individual's needs as outlined by the Individual Habilitation Plan. Accountable for active treatment at all times for assigned individuals within program by ensuring that individuals follow their Individual Activity Schedule and receive adequate supports as outlined by the individual's IHP.
  • Assist in the instruction of individuals inside and outside of the Developmental Training and Work Center (e.g. Situations may dictate Trainer's participation in the residential or other program).
  • Exhibit working knowledge of facility safety / emergency procedures.
  • Documentation and Staffing:
  • Record data to be used in completing progress reports each month for individuals assigned. Complete all required documentation.
  • Document in ID Notes as instructed. Use Communication Log as assigned by Supervisor
  • Establish good rapport with individuals, their families, staff and the community.
  • Attend staff meetings, IHP meetings and inservice training as assigned.
  • Demonstrate basic computer skills for documentation purposes.

  • Communicate and interact with individuals in a positive manner.

  • Report to work as assigned and in a timely manner.
  • Display respect toward supervisor and subordinates.
  • Perform Other Duties As Assigned: Other expectations may be outlined for specific training areas. Completion of additional tasks to meet the needs of the individuals receiving services may be required.
Educational/QualificationsTrainers must have a minimum of a high school diploma or GED. Experience working with developmentally disabled is preferred but not required. In addition, Trainers must be at least 18 years of age, emotionally stable and able to read and write.
  • Willingness to accept supervision and directions and follow specific programming techniques.
  • Ability to effectively communicate verbally and non-verbally.
  • Ability to be a contributing team member and work cooperatively with staff and agency personnel, families, and community.
  • Sufficient physical stamina and mobility for lifting, carrying, pushing, and pulling.
  • Ability to stay calm and not become angry when working with individuals receiving services who are upset.
  • Ability to supervise and train individuals with developmental disabilities in a mature and responsible manner.
  • Ability to read, write and perform basic math skills.
  • Ability to lift as needed for safety and transfer of individuals receiving services.
  • Eligible for insurance by possessing a good driving record.

PurposeSome training areas may require special qualifications. Changes may occur in these qualifications should the type of persons receiving services in the training are change. Current requirements are as follows:
Ability to lift items weighing 30-50 pounds throughout the day.
Basic math skills, to enable completion of payroll/ time studies necessary for the Department of Labor regulations.
Possess basic computer skills to provide accurate documentation.
Ability to assist with wheelchair transfers and ambulation with the person Served.
Working conditions may include exposure to emotionally unstable individuals and require interpersonal skill and/or physical intervention to calm the individual.
Ability to apply physical restraints when needed.
Ability to maintain composure in a stressful environment.
Ability to communicate effectively orally and written.
Be a Team player and step in when you see there is a need.
Valid Driver’s Licenses and ability to drive a van for transportation to and from CDS and community outings.
Perform other responsibilities and duties as assigned by a supervisor.
