Job Description: Baptist Health is looking for a Clinic Specialty Pharmacist (Rheumatology) to join our team at Baptist Health Lexington, Lexington, KY. Communicate with physicians and other clinicians as necessary to facilitate exemplary service for new and existing clinic patients. Enroll specialty patients into the program by assisting with recruitment of patients to ensure the best possible clinical outcomes. Educate clinic and retail pharmacy teams on specialty pharmacy service offerings. Provide support for embedded pharmacy billing specialist(s) and technician(s) where applicable. Address inquiries from clinic staff, providers, patients and families. Patient care services and operations Conduct and support pharmacy team conversion conversations with patients and ensure thorough documentation of achieved outcomes. Provide initial clinical medication counseling and regular patient reassessments to ensure appropriateness of specialty therapy. Perform medication therapy and disease state management, which may include coordinating medication therapy adjustments. Refer appropriate patients to financial counseling for copay assistance through manufacturer or charity programs. Apply principles of continuous quality improvement consistent with job expectations. Communicate with patients and patient families about the benefits of filling their prescriptions with Baptist Health. Minimum Requirements Kentucky and/or Indiana Board of Pharmacy license in good standing. Licensure in surrounding states might be necessary/preferred to fulfill job responsibilities. Bachelor's Degree in Pharmacy or Doctorate of Pharmacy required. 1-3 years related work experience or completion of ASHP accredited pharmacy residency program preferred. Experience in ambulatory clinical pharmacy practice preferred. Disease state specialty experience preferred. Possesses knowledge in drug information, third party claim adjudication and regulatory requirements of pharmacy practices.
