We have an exciting opportunity for you!
We are hiring a Nurse Preceptor to join our Training and Development team. You are a licensed Registered Nurse (RN).
In this role, you will provide supervision to assigned newly hired clinical staff members while serving as the educational preceptor. You will play a critical role in ensuring the successful transition from the date of hire through a 3-month orientation period.
You will precept clinical staff in the following: (includes, not limited to)

  • Patient assessment
  • Hospice Care
  • Palliative Care
  • Care Plan Process with an emphasis on pain and symptom management and specific competency requirements.

    Travel Requirement: This position requires travel throughout the Big Bend Hospice service area, based on the assigned new hires team.

    Duties/Responsibilities:
    (Includes, but is not limited to)

    • Precepts clinical staff, ensures their successful transition from date of hire through 3-month Orientation period.
    • Assists in the development, revision, and successful implementation of classroom orientation for all new employees and clinical staff.
    • Participates in the development, revision and successful implementation of ongoing education and coaching to existing clinical staff.
    • Precepts clinical staff in the following: patient assessment, hospice/palliative care, and the care planning process with an emphasis on pain and symptom management and specific competency requirements.
    • Teaches and facilitates computer documentation (how-to) as well as the required regulatory content of the documentation (what). This includes all aspects of the Electronic Health Record for all applicable disciplines.
    • Role models and conducts point of care documentation, i.e., using the tablet at the bedside, in the home and as an active part of each patient visit.
    • Provides direct patient care periodically to maintain his/her skill sets and competence, as well as maintaining familiarity with the demands of direct patient care.
    • Performs field observation of clinical staff to coach computer use, coaching on how to conduct an efficient model hospice nurse visit, patient assessment, and care planning in hospice/palliative care, IDT model of care/care coordination, clinical/technical skills, and time management skills.
    • Performs field observations as needed for specific identified needs & the initial competency review. Assists team managers with annual competency reviews as needed.
    • Documents clinician’s competency in writing to the Team Manager and Director of Clinical Operations on the progress of using the competency observation checklist.
    • Collaborates with Team Managers to identify clinicians who need refresher training or support. Plans, facilitates and implements that training as well as evaluating its effectiveness and reporting to the Team Manager and Director of Clinical Operations.
    • Facilitates clinical visits of applicable types for new nurses through the team managers: Admission, Comprehensive Assessment, Comprehensive Reassessment, Recertification, Routine, Focus, PRN, On Call, GIP, and CC. Is knowledgeable about each and able to give the required documentation for each applicable visit.
    • Maintains Nursing and CHPN certifications.
    • Conducts CHPN review classes for nurses and aides taking the certification exam on an as-needed basis.
    • Audits patient documentation of clinicians in training and provides feedback and monitoring during the 90-day orientation period to ensure quality and timeliness of documentation.
    • Works directly with the Compliance Department to assist clinicians with a need to improve the quality of their documentation.
    • Demonstrates knowledge, comprehension and compliance with all state and federal laws, rules and regulations, agency policies and procedures, conditions of Participation for Hospice and other required regulatory oversight such as AHCA and CHAP.
    • Plans, organizes, and conducts an annual skills training/check-off day for all clinicians, including providing options for our outer county areas. This training may be adjusted to be completed monthly, quarterly, semi-annually, or annually.
    • Participates in the planning for and orientation of ACOM students.
    • Participates in the orientation of shadowing students to include Hospice 101 presentations at the institutions of learning.
    • Participates in the development, revisions, and successful implementation of ongoing training with BBH facility partners.
    • Other duties as assigned.

    Requirements:

    • Must have an active Florida RN License.
    • Minimum of 2 years of 'hands-on' clinical experience such as providing care, supervising nursing services, or teaching skills in an organized curriculum or in-service program.
    • Must have 1 year of experience in hospice health care.
    • Proficiency in Electronic Medical Records (EMR) and Microsoft Office Suite.
    • Excellent communication and organizational skills
    • Valid driver's license
