Overview

BJC Home Care
offers patients and their families a complete range of home care services, including skilled nursing services, adult and pediatric hospice and supportive care, rehabilitation therapy, home infusion therapy, infusion treatment rooms, home medical equipment and high-tech respiratory care. Specialty home care programs also are available, including adult and pediatric asthma, cardiac, diabetes, orthopedic and wound care programs.
BJC Home Care provides care to thousands of patients in both Missouri and Illinois. Serving more than 25 counties, it has become the largest home care network in the region and one of the largest in the country.
Preferred Qualifications

Role Purpose

The OASIS Coordinator assures that care is provided in accordance with division and health system's values, vision, mission, goals, policies and procedures, and State, Federal, and JCAHO guidelines. Reviews utilization statistics and adverse events, including study of hospitalized patients, seeking patterns and providing correlating opportunities to mitigate preventable risk; setting ongoing expectations for standardized risk stratified, best practice care planning. Responsible for the guidance of best-practice care planning and optimal utilization of service; providing twice a month episode management conferences and education of multi-disciplinary teams in the management of patient care services.


Responsibilities
  • Monitors utilization management patterns to identify clinician, team or agency trends related to high or low visit utilization, relative to benchmark data and patient acuity. Integrates optimal visit utilization expectations, based on clinical acuity and associated management of risk.
  • Recommends additional services or techniques to improve efficiency and effectiveness of cases present in meetings; illustrating optimal, risk-stratified plan of care and to achieve optimal patient outcomes.
  • Serves as a clinical resource to clinical staff and quality department.
  • Identifies and recommends judicious use of resources and cost effective practices without compromising patient care outcomes.
  • Collaborates and strategizes with the quality coordinators and location leaders to improve patient outcomes.
