Job Summary: A clinical pharmacy specialist plays a crucial role in patient care within a healthcare setting, typically a hospital or clinic. Their responsibilities extend beyond those of a traditional pharmacist. Here are some key aspects of what they do:
Responsibilities:
- Serve as a clinical resource to providers
- Case management consultation as requested by providers and other members of the healthcare team
- Completion of population health initiatives (e.g., drugs to avoid in the elderly, HEDIS, drug interaction)
- Serve as a vaccine coordinator for the practice (ensure compliance with government and Bloom vaccine policies and procedures)
- Present clinical education to providers and team
- Create clinical resources for providers and team
- Precept pharmacy students
- PharmD or BSPharm
- Current Colorado pharmacist license in good standing
- Completion of PGY1 and/or PGY2 pharmacy residency
- BCPS, BCACP, BCGP, or BCPP (at hire or within 3 years of hire date)
- BLS
- Immunization certification
