Blue Ridge Health is currently seeking a Behavioral Health Case Manager to be a part of our Behavioral Health Team.
The case manager will provide case management services and support for participants to increase the client’s ability to successfully navigate their lives as independently as possible. This will include making home and community visits to assist consumers with access to, and engagement in, necessary mental health/substance use (MH/SU) services, care coordination between two or more agencies, including medical and non-medical providers; basic Needs (housing, food and nutrition, transportation) or skill Development (including self, interpersonal, and crisis management).
