Description

Essential Job Functions: 

1. Provides direct patient care as directed in the plan of care.
2. Implements current nursing practice following a comprehensive assessment and the plan of care.
3. Provide accurate and timely documentation of patient services to reflect the plan of care. Documentation is entered the EMR at bedside. 
4. Provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care as directed by the RN or provider.
5. Uses equipment and supplies effectively and efficiently. 
6. Provides appropriate pain/symptom management. Evaluates patient’s response to treatments/medications.
7. Participates in personal, professional growth and development. Also participates in quality assessment performance improvement teams and activities, and peer supervision. 
8. Attends IDT meetings. 
9. Performs other duties as assigned by manager or team leader.
10. Must be a licensed driver with an automobile that is insured in accordance to state or organization requirements and is in good working order. Must be willing to drive the entire service area.

Qualifications

Physical/Sensory/Cognitive Requirements: The requirements described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. 

1. Must be able to continuously stand and walk, frequently squat, climb stairs, bend to the floor, reach over the shoulder, kneel, twist, sit, lift patients, and remain in uncomfortable positions for prolonged periods of time. 
2. Must have visual acuity enough to continuously note changes in patient condition (breathing, color, and skin integrity) and to read a thermometer, sphygmomanometer dial, and other instruments. 
3. Must have hearing acuity enough to auscultate BP, hear verbal instructions and communication from clients, respond to emergency alarms, and answer the telephone.
4. Must be able to lift up to 50 pounds.
