A reputable community health system in New Hampshire is searching for a Complex Care Registered Nurse for one of its clinics in Somersworth. In this role, you will provide patient-centered complex care in collaboration with the Primary Care Physician, Medical Assistant, and multi-disciplinary team members, ensuring the safe and accurate delivery of care. Responsibilities:

• Providing direct care to patients requiring complex care.
• Effectively triaging patients.
• Gathering and interpreting data through history taking and physical assessments and planning interventions to meet patients’ needs.
• Documenting the care provided and the patient’s progress toward desired outcomes.
• Educating patients on how to properly manage their care at home.

Qualifications:

• A BSN is preferred.
• Active RN License.

• Full-time, permanent position
• Monday-Friday, 8a-430p
• NO WEEKENDS!
• Pay: $31-36/hr., depending on experience.
