Responsibilities:
- Provide case management services to patients, ensuring their healthcare needs are met
- Conduct patient assessments and develop individualized care plans
- Collaborate with interdisciplinary healthcare team members to coordinate and facilitate patient care
- Perform diagnostic evaluations and monitor patient progress
- Utilize knowledge of physiology and medical conditions to provide appropriate care and interventions
- Implement behavioral health strategies to support patients' mental and emotional well-being
- Utilize utilization management techniques to optimize healthcare resources and ensure cost-effective care
- Provide patient education and support to promote self-care and disease management
- Coordinate discharge planning and ensure smooth transitions of care

Requirements:
- Valid Registered Nurse (RN) license in the state of [location]
- Proven experience in case management or a related field
