JOB SUMMARY
As a CHHA, your role will involve providing personalized skilled nursing care to homebound patients through customized care plans that prioritize health and independence. Effective communication skills are crucial for collaborating with medical professionals, patients, families, and the care team. DUTIES
- Aid clients in daily tasks like grooming, dressing, and bathing
- Prepare meals and assist with feeding as needed
- Offer companionship and emotional assistance
- Execute light housekeeping, organizing client's space
- Adhere to healthcare professionals' care plans and guidelines
- Monitor and report changes in client's condition
- Support medication reminders and administration
- Maintain precise medical records

QUALIFICATIONS
- Certification: Possession of a valid California Home Health Aide certification
- Background Check: Successful completion of background checks
- Health Screening: Clearance of a physical examination to ensure the ability to perform essential job functions
- CPR Certification: Current certification in Cardiopulmonary Resuscitation (CPR)
- Communication Skills: Strong communication abilities to interact effectively with clients, families, and healthcare professionals
- Compassion: Demonstrated compassion and patience when working with individuals with special needs or health challenges
- Reliability and Punctuality: Dependable and punctual attendance for assigned shifts and responsibilities
- Documentation Skills: Proficiency in maintaining accurate records, documentation, and reporting client information
- Basic Medical Knowledge: Familiarity with basic medical terminology and understanding of healthcare procedures
- Adherence to Care Plans: Ability to follow care plans and instructions provided by healthcare professionals accurately
- Medication Assistance: Competence in providing medication reminders and, if necessary, assistance with administration
