Job Description:

We are seeking skilled and compassionate Registered Nurses to join our Hospital at Home Program team. We work together with physicians and other healthcare system partners to provide patients with hospital level care in the comfort of their own home. The ideal candidate will have experience in medical-surgical nursing, proficiency in performing blood draws and IV placement, expertise in medication management, and excellent communication skills to liaise with physicians effectively. This per-diem position offers flexible scheduling with Monday through Friday blocks available from 7:00 am to 12:00 pm and/or 4:00 pm to 9:00 pm.

Key Responsibilities:

- Provide high-quality nursing care to patients enrolled in the Medically Home Program

- Perform blood draws and IV placements with precision and care

- Manage medications for patients according to prescribed regimens

- Communicate effectively with physicians to ensure coordinated patient care

- Document patient assessments, interventions, and outcomes accurately and timely

We ensure that our RNs have the skills they need to be successful in caring for our clients through competency demonstration in our on-site simulation lab. We also offer client-specific skill enhancement opportunities in the home when indicated. Necessary PPE is provided so that you can feel confident and safe when you care for your clients.

Qualifications/Requirements:

- Active Registered Nurse license in the Commonwealth of Massachusetts

- Must have or able to obtain BLS CPR Certification

- Minimum of 1 year of documented experience in medical-surgical nursing

- Proficient in blood draws and IV placement

- Strong medication management skills

- Excellent interpersonal and communication skills

- Recent wellness exam and negative TB skin test (provided if needed)

- Ability to pass a criminal background check and drug screening

- Ensures confidentiality and security of the client’s medical information

- Must be able to stand, walk, squat, bend, kneel, reach, twist, push and pull. Varied assistance is required depending on the mobility of the client. Must be able to life up to 50 pounds.

- Ability to work independently and collaboratively within a multidisciplinary team
