Position Summary The primary purpose of your position is to schedule and coordinate home care services in accordance with participant care plans, ensuring maximum productivity of home health aide schedules, communicating schedule changes to staff, participants, and home care coordinators. This position provides coverage in the absence of a home care coordinator , administrative supports to the home care department, processes service utilization approvals for home care hours, DME, and supplies. Actively engages in inclement weather activities and provisions.

Required Qualifications
LPN required with 3-5 years Home care experience required
One year of experience working with the frail elderly
Must have reliable transportation and a current valid Michigan driver’s license.
current CPR certification by AHA or be willing and able to obtain as offered by LifeCircles.
Strong desire to work in a team based model of care
Organized and have excellent time management skills
Advanced skills in Excel, Microsoft Office, Outlook, and electronic medical records
strong sense of professionalism, good critical thinking, creativity, flexibility, and excellent problem-solving skills
