Provides supervision to Hospice team of registered nurses, medical social worker, aides, bereavement coordinator, office staff. Staff provides services to Hospice patients in the home, skilled nursing facilities and inpatient. Submits required documents to payers, oversight of timely filing of HIS, acts as a resource to all staff related to Hospice and Palliative Care. Participates in operations, quality and other committees, projects and other tasks to support Agency needs. Manages payroll, human resources for Hospice team and all other clinical coordinator responsibilities. Makes on site supervisory visits with staff to observe competencies. Stays current regarding Hospice regulations, industry trends and issues. Actively participates in DPH, JCAHO,Baldrige, Magnet surveys,and audits, Responds to patient and families needing additional information about services and acts as a model of professionalism and customer service to her staff. This role will also manage a small caseload of patients.
Qualifications
Educational / Minimum requirements
Master's degree from a program approved by the National League for Nursing or the American Public Health Association with a minimum of one (1) year full time clinical experience in a hospice agency or related community health program which included hospice OR:
A baccalaureate degree in nursing with a minimum of three (3) years full time clinical experience, at least one (1) of which was in hospice or a community health program that included hospice, OR;
A diploma in nursing or an associate's degree in nursing AND
A minimum of three (3) years of full time clinical experience in the last five years, at least one of which was in hospice, AND
Evidence of certification by the American Nurses' Association as a community health nurse or completion of at least six (6) credits in health care management from an accredited college or university program or school of nursing.
State/Federal Mandated Licensure or Certification Requirements:
CT Licensed RN
CT Drivers license
Bristol Hospital Mandated Educational Requirements:
General orientation at time of hire. Fire/Safety/Infection Control annually. Other programs as mandated by Hospital.
Special Requirements:
Experience in Hospice/Palliative Care, strong assessment skills, interpersonal skills, management experience
Physical Requirements:
Exposure to extended walking and standing.
Work Environment:
Exposure to patient elements; exposure to corporate client work sites.
Cognitive Requirements:
Basic math skills, ability to accurately document results of patient encounters.
