RESPONSIBILITIES
Manage a Diverse Caseload: Oversee a full caseload of clients from various backgrounds, ensuring tailored and effective support for each individual.
Facilitate community workshops both in-person and on virtual platforms as dictated by the supervisor.
Maintain familiarity with public benefit and entitlements.
Conduct Intakes and Assessments: Perform daily intakes and assessments, accurately entering data into the database.
Provide Referrals: Offer both internal and external referrals to clients based on their specific needs.
Client Advocacy and Follow-Up: Advocate for clients and provide follow-up services to ensure continued support and progress.
Document Client Progress: Maintain detailed records of client progress and all interactions, ensuring accurate and up-to-date documentation in the database.
Assist with Applications: Help clients complete applications for government benefits and other necessary services.
Community Outreach and Support: Participate in community outreach initiatives and attend scheduled agency food pantries.
Direct Client Services: Deliver comprehensive services to clients, including intake, engagement, assessment, service plan development, referrals, advocacy, exit summaries, and follow-up.
Specialized Program Management: Manage caseloads for specific programs such as the NYC Benefits and Food Access and Benefit Initiative programs.
Monitor Service Plan Goals: Regularly track and document client progress toward achieving service plan goals.\
Resource Familiarity: Stay informed about available program resources both on-site and off-site.
Community Engagement: Conduct presentations and participate in community events to raise awareness and provide information about available services.
Service Coordination: Coordinate services with other providers, including social services, mental health, medical, employment, educational, and childcare providers.
Case Conferences: Hold regular case conferences with clients and service providers to review and adjust service plans as needed.
Program Reporting: Complete and submit program reports on a monthly basis or as required.
Supervisory Meetings: Report to and meet with your supervisor on a monthly basis to review caseloads and discuss any issues or progress.
HIPAA Compliance: Maintain strict adherence to HIPAA compliance standards, ensuring the confidentiality and security of all client information.
Funder Compliance: Ensure compliance with all funder requirements, including reporting, documentation, and service delivery standards.
Additional Duties: Perform other duties as assigned to support the overall mission and effectiveness of the agency.
