Practice and promote the agency’s mission statement in every interaction. Provide ongoing support to people in becoming active participants in every aspect of their lives and to ensure that a high quality of life is maintained. Performs initial Physical Therapy evaluations and completes evaluations and all follow-up evaluations. Provides direct PT intervention as needed, develops appropriate PT programs. Completes temporary service plans, 21-day reviews, and quarterly and annual reviews. Provides staff training as needed.
