HRL Care CoordinatorJob No : 541520Work Type : StaffLocation : Long BeachCategories : Unit 4 - APC - Academic Professionals of California, Administrative, Temporary, Full TimeJob Summary This position reports to the Director of Support and Prevention Services, which serves as one of the mental health supports on campus.
The incumbent will provide case management and care coordination to high-risk students with psychological and social concerns, working in conjunction with on-campus Health & Wellness resources, who are primarily housed within Housing and Residential Life (the dorms).
The incumbent will collaborate with campus partners such as Bob Murphy Access Center, Title IX, Student Health Services (SHS), Basic Needs, CARES, CAST and related programs, and other areas as assigned.
The incumbent provides a diversity, equity, inclusion, and accessible approach to care offered to the campus community.Furthermore, the incumbent provides services that are equitable, inclusive, and culturally sensitive to enhance the quality of campus life.

Key Responsibilities Mental Health Case Management - Provide case management and linkage to housing students referred by HRL, CAPS, and CARES or other campus partners.
Manage a student caseload, providing coaching, support, and advocacy. Provide case conferencing with staff from BMAC, CARES, CAPS, HRL, and other campus partners.
Care Coordination and Campus Collaboration - Provides on-going coordination of care for students who are in housing to ensure they remain linked to those resources.
Provide care coordination of services to students on a walk-in and scheduled basis; refer students to outside agencies, on and off campus partners, and services when appropriate.
Provide additional support to campus mental health programs, as needed. Knowledge Skills and AbilitiesKnowledge of Fair Housing Act, FERPA, and HIPAA.
Knowledge of mental health and social services for college students. Ability to maintain confidentiality and sensitivity to confidential issues in all communications.
Ability to communicate and work effectively with an ethnically and culturally diverse campus community. Ability to apply independent, reasoned judgment, discretion, and initiative to address problems and develop practical, thorough, and technically accurate solutions.
Ability to work under pressure, maintain equanimity, and remain calm and focused, as well as calming and focusing others in an extremely fast-paced work environment.
Excellent oral and written communication skills, including ability to make effective presentations, facilitate an open exchange of ideas, and write clear, concise notes, documents, and reports.
Strong computer skills with experience with a broad range of Microsoft Office Suite (including Excel, PowerPoint, etc.) to be able to prepare a wide variety of correspondence, reports, and presentations.
Ability to communicate with an ethnically and culturally diverse campus community. Ability to follow all university policies, procedures, and guidelines including but not limited to safety, civility, information security, and non-discrimination policies and procedures.
Ability to contribute to a positive university experience for each and every student and assist in achieving the university's commitment to a "vision of excellence.
Education and Experience Equivalent to a bachelor's degree with upper or graduate coursework in counseling and guidance, human relations, or related fields and four years of professional experience in one of the student services program areas required.
Preferred : Master's degree in Counseling, Social Work, or related field. Professional / Clinical licensure or currently working towards licensure.
Two years of experience providing services to vulnerable populations.Physical Summary Medium work - Job involves lifting heavy-weight objects (20-40 pounds) and / or 20% - 40% of the job involves standing, squatting, kneeling, or walking.
May require pushing or pulling objects within the weight limits. Licenses / CertificatesPossess a valid California Driver's License to drive University vehicle.
Employees who are required to operate motorized vehicles and / or use their personal vehicle more than once a month on university business are required to take a mandatory Defensive Driving course and enroll in the DMV Employee Pull Notice (EPN) Program.
