Job SummaryThe Director of Care Coordination is responsible for the oversight and management of the medical records and referrals department. They manage and secure all written and electronic data within a facility's medical records department and ensure that information contained in each record is complete, accurate, and secure. Furthermore, they must monitor the referrals department to guarantee that all referrals satisfy the medical needs of our patients and providers for their continuity of care. This position requires expertise in a systems approach to problem solving, using critical thinking skills and knowledge of community resources to plan and coordinate care.
Essential Duties & Responsibilities
  • Responsible for coordination of care across the continuum and population health management, for low, medium, and high-risk patients.
  • Provide coordination of care and services through the management of a team of care coordination staff.
  • Interpret and direct the implementation of departmental policies and procedures and apply recognized standards of care and practice.
  • Plan, select, and devise care coordination operations, work methods, procedures, workflow and standards for quality and quantity of work, including staffing standards.
  • Establish, implement, and evaluate quality of care. Plan and develop patient safety innovations and practice improvement.
  • Performs and participates in analysis, data, insurance, member and progress reports within a timely manner.
  • Perform other related duties and participate in special projects as assigned.
  • Recognize and respond to patient care issues providing direction, feedback, and follow-up to staff or other healthcare team members.
  • Plans, assigns and reviews work of staff to ensure that group objectives are met.
  • Verify that all regulatory compliance with HIPPA, OSHA, labor laws, and other federal, state and local regulations are appropriately proceeded.
  • Ensures patient satisfaction, including troubleshooting where there is a grievance or complaint and developing process improvements to prevent recurrences.
Supervisory Responsibilities
  • Directly supervise the department managers and provide oversight, leadership, and guidance for the care coordination staff.
  • Conduct monthly staff meetings to review progress on compliance measures, discuss any changes in methodologies, review any new policies and procedures, and to address other issues as appropriate.
  • Hires, trains, develops, and manages staff to ensure that a productive team exists to meet group objectives and conducts disciplinary manners and performance evaluations.
Education & Experience
  • Minimum of three to five years of supervisory or management experience. Minimum of five years in a medical staff environment.

Knowledge, Skills & Proficiencies
  • Extensive knowledge of credentialing and care coordination, the use of state-of-the art management systems, with knowledge of eCW systems preferred.
  • Strong demonstrated ability with solid practical experience to design and implement systems for data collection and reporting.
  • The ability to organize, set priorities, communicate, work independently, and work well with a diverse group of people is essential.
  • Knowledge of regulations for all insurance types, HIPPA and labor law.
  • Multi-tasking, problem-solving, customer service and presentation skills needed.
  • Excellent leadership abilities with demonstrated communication and writing skills essential for this position.
Physical RequirementsThis position works under usual office conditions. The employee is required to work at a personal computer as well as be on the phone for extended periods of time. Must be able to stand, sit, walk and occasionally climb. The incumbent must be able to work extended and flexible hours and weekends as needed. Physical demands include ability to lift up to 50 lbs. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of the job.
