Facility Cape Fear Valley Medical Center Location Fayetteville, North Carolina Department Paramedic Mental Health Program Job Family Professional Work Shift Days (United States of America) Summary The SWIII plans for services for the targeted population. Responsible for psychosocial assessments, crisis intervention, discharge planning, and coordination of referrals and resource information to patients and families in need of assistance. Independently identifies a complex patient as well as receives referrals from nursing and other ancillary staff. Provides an array of social work services to patients and families to promote understanding and resolution of problems related to environment stress, physical illness, interpersonal conflicts, and other psychosocial issues. Adheres to the patient experience initiatives and champions customer service. Works collaboratively to ensure patient needs are met and care delivery is coordinated across the continuum at the appropriate level of care. The SWIII is also the Clinical Supervision Provider, is a board-approved Licensed Clinical Social Worker, that provides clinical supervision to provisional Licensed Social Worker. The clinical supervision provider is responsible for providing direction to the supervisee, who applies social work theory, standardized knowledge, skills, competency, and ethical content. ADDITIONAL RESPONSIBILITIES Participates in weekend and holiday rotation as required. Serves as preceptor for newly hired Social Worker (I, II) as requested by the leadership Team. Provides Clinical Supervision to Provisionally Licensed Social Worker QUALIFICATIONS The following qualifications are requirements necessary to perform the essential functions of this job. EDUCATION AND FORMAL TRAINING Master’s degree in Social Work (MSW) with Current License in North Carolina (LCSW) WORK EXPERIENCE Minimum of 1 year experience in Social Services, Home Health, Hospice or Acute Care setting preferred. Acute Care Case Management preferred. Previous community health experience preferred. Must have knowledge of federal regulations and community resources. Written and verbal communication skills essential. KNOWLEDGE, SKILLS AND ABILITIES REQUIRED Must be flexible with scheduled hours to meet the needs of the department, unit, patient and family. Work is not standardized and requires a high degree of prioritization skills. Excellent interpersonal communication and negotiation skills. Able to communicate effectively and work with people of all social, economic and cultural backgrounds. Self-motivated, proven written, telephonic and electronic communication skills, assertive and persuasive in interactions with customers, peers, management and core staff served. Strong organizational and time management skills, as evidenced by capacity to prioritize multiple tasks and role components. Proficiency with various computer programs, to include Microsoft Office, EPIC, eHIM, Teletracking and SMS Skills necessary to build credible and effective relationships with physician leadership, the internal and external customer. Ability to demonstrate respect and team building, Flexible, open minded and adaptable to change. Models positive change. Ability to work and promote interdependence demonstrated in actions resulting in sound judgement with interactions with physicians, peers, patients and/or designee, payors, support service personnel. Ability to work collaboratively with department staff, physicians and healthcare professionals at all levels to achieve established goals, improve quality of outcomes, maintain, or exceed Joint Commission standards and State mandates as they apply to the department operations. PHYSICAL REQUIREMENTS Some light carrying and lifting may be required. Occasional walking may be required to access all areas of the Medical Center. Ability to effectively communicate orally to patients, family members, personnel and physicians. Near visual acuity to proofread hand or typewritten materials. Manual ability to use telephone and computer keyboard.
