Hours/Shifts: M-F 9-5, Hybrid

Job Description:

We are seeking a dedicated and experienced Case Worker to join our team in the Bronx, NY. This is an in-hospital only role with no home visits. The ideal candidate will have a minimum of 1 year of experience in case management or social services and must hold a Bachelor's degree.

Key Responsibilities:

• Conduct interviews and make initial assessments and referrals.
• Manage cases of social service recipients, ensuring they receive the appropriate health-related and social services.
• Investigate and verify information obtained from clients.
• Provide support and resources to clients to address their needs.
• Maintain detailed records of client interactions and case progress.

Qualifications:

• Bachelor's degree required.
• Minimum of 1 year of experience in case management or social services.
• Strong communication and interpersonal skills.
• Ability to conduct thorough assessments and make accurate referrals.
• Experience working in a hospital setting is preferred.
• Knowledge of health-related and social service resources.
