Care 4 U Community Health Center is Hiring a Lead Navigator Near Miami, FL
- Coordinating the day-to-day activities of the program, ensuring services are coordination between C4U’s departments and community partners.
- Meeting with project partner(s) and community resources to coordinate program activities and develop a resource directory of care, prevention and support services available to programs participants
- Assist clients in completing grant required questionnaires and surveys.
- Linking clients to needed services and following up on referrals to ensure clear communication and coordination between the project and external providers.
- Recruit members from the target community to serve on the Advisory Planning Committee that will provide input on services to be provided and evidence-based prevention strategies to implement to ensure services are appropriate for the target population.
- Attending relevant community planning meetings to ensure program activities are coordinated with any local or state-wide planning initiatives.
- Assisting the Program Director with data collection and program reporting
- A bachelor's degree in social services, public health or related field or at least five years’ experience providing or coordinating HIV and/or substance abuse programs required.
- Strong understanding of the target LGBTQ population.
- Knowledge of community resources for the target population
- Strong writing, communication, and computer skills.
Care 4 U Community Health Center is a nonprofit, comprehensive community health center, providing a full range of health care and preventative services to our community’s adult, adolescent, pediatric and specialty needs populations. Care 4 U's operates with the mission to eliminate barriers to accessing primary care by expanding medical care services to reach low-income individuals regardless of their ability to pay. The health center provides primary medical care for men, women, and children, including well-child visits; immunizations; HIV specialty care; and STD screening. Care 4 U also provides outreach, case management/linkage to care services, on-site pharmacy, mental health and substance abuse counseling, transportation and emergency financial assistance services to low-income individuals in the high-need area of Liberty City.
