Position Summary: Applicants must reside in the state to qualify.
Care Hawaii is looking for a compassionate and dedicated Case Manager to join our team. As a Case Manager, you will be part of a multi-disciplinary team providing direct support and intervention to individuals experiencing a mental health crisis. Services are delivered on-site in a secure crisis center.
Responsibilities:
- Conduct assessments
- Implement crisis intervention techniques
- Coordinate care with other team members
Qualifications:
Education:
- Bachelor’s degree from a nationally accredited college or university in counseling, social work, nursing, or a related field
- OR licensure or eligibility for licensure in psychology, social work, substance abuse, nursing, or related fields (e.g., LPN, RN, CSAC, LCSW, LMFT, CPRP)
Experience:
- Excellent communication and clinical skills
- Competence in problem-solving, critical thinking, collaboration, and clinical competencies
- 1 year of experience in a mental health and/or healthcare setting
Certifications/Registrations:
- CPR and First Aid (prior to or after start)
- TB
- Background Check
- CPI (training provided upon hire)
- Valid Driver’s License
- Current and valid vehicle documentation
