Physical Therapist
Care IV Home Health, a locally owned and operated home health agency, is seeking a full-time Physical Therapist to join our esteemed team in Little Rock. Physical Therapist Responsibilities:
Conduct visits to patients within our Little Rock service area, delivering care in their homes or assisted living facilities
Design and implement individualized physical therapy care plans
Document patient progress, including admissions, discharges, OASIS assessments, and regular updates to the Director
Fulfill any additional duties assigned by the Director
Physical Therapist Qualifications:
Visit patients in the comfort of their homes and assisted living facilities in our
Active licensure or registration as a Physical Therapist in the state of Arkansas
Preferably a minimum of six months of prior PT experience but not required
Capability and willingness to travel frequently to patient homes
