Home Health RN Case Manager

Full time (Office and Outside patient Visits)

Rate: $35 to $50

Job Description and Duties Required: 

· Open New Cases/ SOC for Wound Care, Wound Vac, Medication Management, G-tube Feeding, Tracheostomy Care, Colostomy Care, Resumption of Care, Recertification of Care, Discharge of care, Routine Nursing etc…..Nurses will work in the office and outside the field within a week, Mileage reimbursement. Supervise and train LPN and RN in Tracheostomy, Oxygen treatment, colostomy care, dressing change, feeding pumps, seizure precaution, cough assist, and Foley catheter maintenance. Training will be provided. Monitor patients' conditions and assess their needs to provide the best care and advice. Nurses will work with the Care Coordination Team to be successful in all patient care.

· Assess and evaluate patient status, develop and initiate personalized care plans, and make necessary revisions as patient needs evolve. Collaborate with Physicians & nurses to devise individualized care plans for patients.

• Properly complete OASIS assessments and submit required documentation to meet both state and Medicare standards. QA on all clinical charts and documentation.
