• Completes an initial assessment of patient and family to determine home care needs
• Conducting supervisory visits for LVN and CHHA
• Provides a complete physical assessment and history of current and previous illness(es)
• Initiates the plan of care and makes necessary revisions of patient status
• Develops a care plan, which establishes goals based on nursing diagnosis
• Coordinates with interdisciplinary team to develop a plan of care to meet patient/family/caregiver needs
• Delegates, monitors and supervises paraprofessional staff and makes adjustments to assignments as needed.
• Experience in wound vacs, PICC care, IV therapy
• Experienced in Home care home base (HCHB) and OASIS

Job Type: Full-time

Pay: $95.00 - $140.00 per hour

Expected hours: 10 per week
