- Perform home visits for patients in a home care setting.
- Regularly re-evaluates the therapy needs of our clients; initiates, develops, implements, and makes necessary revisions to our patient's plan of care.
- Observes, records, and coordinates care with multi-disciplines regarding the patient's treatment and any change in the client's condition.
- Teaches, supervises, and counsels the patient and family regarding home therapy procedures and other care needs as appropriate to the patient's condition.
- Current license as an Occupational Therapist in the state of Connecticut.
- Valid Driver's License
- Reliable transportation and willing to travel in New London County and other surrounding towns.
- Strong communication and interpersonal skills.
- Home health experience including Medicare preferred.
- Flexible schedule.
- Mileage paid between patients visits.
- Training, professional development and technology.
- Benefits including time off, health insurance, and agency vehicle are dependent on the number of visits worked each week.
