Description
The ESSHI Case Manager is responsible for supporting clients in maintaining stable housing. The position provides support to community members who have experienced episodes of homelessness and works to ensure clients have access to high quality support services. Activities conducted by this position fall within the following three categories: 1) Care coordination and support service referrals 2) Individualized service plan development and review and 3) Administrative program oversight and regulation compliance.

? Experience with MS Word, Outlook, Excel, OneDrive, and SharePoint preferred.
? Must possess excellent communication skills, both orally and in written form.

Full time role, 40 hours per week. Monday through Friday with hours flexible between 7:30am-6:00pm
