Job Summary

Caregivers follow the individual care plans for each client, professionally assisting them with activities of daily living, instrumentals of daily living and related household activities. This assistance helps clients stay in their homes, facilities, or other settings. Caregivers work dayshifts, nightshifts; 6 hours, 8 hours, 12 hours, 24 hours, or live-in arrangements. Weekdays, Weekends, Holidays. Shifts less than 6 hours may be offered.

Job Requirements and Essential Job Functions

  • Capably and kindly assists clients with activities of daily living (ADL), such as toileting, incontinence care, bathing, mobility, grooming, dressing, and feeding.
  • Carefully and efficiently performs instrumental activities of daily living (IADL) for clients, such as performing light housekeeping duties, preparing meals, doing laundry, providing medication reminders, transporting clients to appointments, going shopping, and providing daily companionship.
  • Safely assists clients in and out of wheelchairs, vehicles, beds, furniture, and other spaces.
  • Carefully reviews and follows the individual client care plans, while encouraging clients to be as independent as possible.
  • Document completion of daily tasks through an online system or as instructed in a CHOS company care binder.
  • Maintains professional boundaries with clients and family members; being friendly and personable, yet not crossing professional boundaries.
  • Strictly maintains the confidentiality of clients’ Protected Health Information (PHI), and complies with all federal and state patient privacy laws.
  • Acts courteously and professionally when interacting with staff at residential facilities and others in the community with whom the company conducts business.
  • Acts considerately and civilly in interactions with coworkers and management, offering to assist when needed, and being a contributing member of the CHOS team.
  • Successfully works independently with minimal supervision and as a team.
  • Consistently follows dress code, is prompt and reliable in reporting to work on time, and complies with all other company policies.
  • Dependably works shifts as assigned and agreed upon in advance.
  • Effectively and clearly communicates, both verbally and in writing, including in the use of CHOS electronic communications and scheduling tools.
  • Drives safely and carefully, and maintains a clean driving record which allows for insurability.
  • Promptly informs administration of client requests, concerns, schedule or care routine changes, and outside service companies; as well as other issues that must be addressed from the office.
  • Follow the company chain of command.
  • Attends and participates in team meetings, staff development training, and care plan reviews.
  • Other duties as assigned.

Minimum Education, Experience, and Qualifications:

  • High School diploma or equivalent is required.
  • Professional caregiving experience is preferred.
  • Successful results on pre-employment drug test and background checks are required.
  • Must possess a valid driver’s license, have reliable transportation, and provide evidence of insurability with a clean driving record.
  • Must obtain CPR/First Aid certification within 30 days of hire.
  • Must have basic computer skills.
  • Must have a Smartphone
  • Must be eligible to work in the United States, and provide documentation for Form I-9 and E-Verify.

Physical Requirements and Work Environment:

  • Work time is spent in clients’ homes, residential settings, and facility settings which are temperature-controlled environments; transporting clients to appointments using an employee’s vehicle or client’s vehicle, performing errands, and shopping duties.
  • Must be able to perform all ADL and IADL duties, and safely utilize and maneuver Hoyer Lift.
  • Duties include frequent sitting, standing, lifting and carrying up to 50 pounds, bending twisting, and reaching.
  • The ability to perform stand-by assistance and partial assistance is required.
  • Must be able to effectively communicate orally and in writing with others in-person, by phone, and through electronic devices.
