The Community Stabilization Service Coordinator position is responsible for ensuring the coordination and provision for individuals enrolled in the Caritas Family Solutions Community FamilyStabilization Program case management services.

This position will work as the liaison between community resources and the recipients.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following :

Serves as the primary point of contact for all communication for assigned families in the community, including St. Clair County Coordinated Entry System for potential new admissions.

Reports directly to the Executive Director of Community Services (EDCS).

• Coordinates, schedules, and leads collaborative family meetings. Meets regularly with the EDCS for planning and strategizing.
• Advocates for the educational, medical, social, emotional, and physical needs of youth and enrolled family members to the various members of family’s teams.

Works with Director of Nursing for applicable nursing and medical needs for family.
