Carolina Caring is looking for a dedicated Nurse Practitioner to join our Home-Based Primary Care program in Mecklenburg County. In this role, the Nurse Practitioner will assess and manage patient, and caregiver needs to deliver comprehensive primary care in the comfort of patients' homes.

This position requires the ability to travel throughout Mecklenburg County to provide in-home care.

Education:

Graduate of an accredited school of nursing with a master’s degree in nursing and appropriate advanced training required for licensure as a registered nurse practitioner. Current, unrestricted North Carolina Nurse Practitioner license. Must maintain CPR certification. Must have active DEA certificate.

Experience:

A minimum of two years in a Primary Care setting. Gerontology experience or certification preferred. Experience with CMS Chronic Care Management Services preferred.
