JOB SUMMARY

Under the direction of the Home and Community Based Care Supervisor, the MnCHOICES
Assessor Social Worker is assigned to the MnCHOICES Unit as a MnCHOICES Certified Assessor. The MnCHOICES Assessors performs high level social work or nursing involving assessment and eligibility determination for publicly funded long-term services and support programs for persons with disabilities. Duties and responsibilities include conducting comprehensive online MnCHOICES Assessments for individuals assessing their physical, mental, social, and environmental health status.
TYPICAL DUTIES AND RESPONSIBILITIES

• Performs individual screening and intake functions and activities for the division or program area.
• Performs and conducts a variety of activities, tasks and functions associated with treatment planning, evaluation, and assessment.
• Determine applicant's risk of institutional care and level of care according to state and federal criteria.
• Implements and provides professional social/human services to individuals, families and/or the community.
• Analyze assessed data and assist applicants in identifying specific services and supports to enhance their abilities to meet goals and live independently in the community.
• Maintain broad knowledge base related to payer sources and community-based services.
• Collaborate with other professionals, providers, therapists, health professionals, internal departments, law enforcement, community, or governmental agencies in coordinating resources and in meeting the needs of consumers.
• Determines eligibility for home and community-based programs (BI, CAC, CADI, DD waivers and PCA) based on individualized assessed needs.
• Coordinate the provision of long-term services and supports to ensure appropriateness, quality, and cost effectiveness.
• Maintain working knowledge of State and Federal laws and regulations and perform work within parameters.
• Performs other comparative duties of a like or similar nature as assigned.

MINIMUM QUALIFICATIONS
•BA or BS in Psychology, social work, sociology, or related area.
•Minimum of 2 years previous experience in human/social services.
•Requires a valid MN driver's license or evidence of equivalent mobility.
