Prism Health is the only nonprofit LGBTQIA health center providing comprehensive primary care, behavioral healthcare, and pharmacy services in the Pacific Northwest (Oregon, Washington, and Idaho). Since opening its doors in 2017, Prism Health has become a model for culturally specific care and now serves over 1,500 patients with a wide range of services, from day-to-day primary care and pharmacy needs to targeted services like gender-affirming care and specialized population health screenings. Prism a program apart of Cascade AIDS Project is excited to announce that we are hiring skilled and team-orientedOutreach Worker/QMHA for Trans / Gender Diverse Services. The Outreach Worker/QMHAprovides services to transgender identified individuals enrolled in Primary Care or Behavioral Health services who have been identified as having case management needs. The Outreach Worker/QMHA works to connect individuals to resources, promote greater independence, community integration, and recovery. This position will work collaboratively with the Peer Support Specialist to help resolve barriers and connect individuals with resources in the community.

This is a Full-Time role working at our Prism Morris location in NE Portland The schedule for this role is Monday-Friday 8:30am-5:00pm. This position includes travel throughout the service area for meetings and client home-visits. Must be available to work evenings and weekends for required agency events.

Starting pay is $52,083.00. The Outreach Worker/QMHA’s pay increases with tenure and the top of the pay scale is $59,883.00. This is a union represented position, so the compensation, benefits and conditions of work are collectively bargained.

Who You Are

  • QMHA credential through MHACBO 
  • Bachelor’s degree in behavioral sciences field (social work, public or community health, psychology) or one year of relevant work, education, training, or experience.
  • Knowledgeable about the lived experience of experiencing mental health and/or substance use issues and willingness to mindfully share those experiences.
  • CADC through MHACBO
  • Experience working with OCHIN Epic electronic health record (EHR).
  • Successful experience working with ethnic, racial, economic, and sexually diverse populations and persons who have experienced homelessness, persons with a mental illness and/or substance abuse.

What You’ll Do

  • Work in collaboration with Prism Behavioral Health team, Providence Health & Services, medical providers, service navigators, or other providers involved in client care.
  • Uses a Person-Centered approach to create and work on treatment goals tailored to the specific needs of the individual receiving services.
  • Uses own experiences as guided by training and models the principles of recovery for those they work with, including gaining greater independence, emphasize wellness and self-determination.
  • Accompany clients to appointments to facilitate access to medical care, substance use treatment, mental health services, housing, and other services.
  • Assists the persons they work with to identify and utilize natural community supports to decrease dependence on the traditional mental health system.
  • Provide support to clients who are having symptoms that could result in hospitalization. Duties could include grocery shopping, meal preparation or just spending time with clients until they feel secure enough to be on their own.
