Position: Behavioral Health Care Manager

Location: Garlington Health Center, NE Portland.

Schedule: Monday through Friday, typically 8:30 a.m. to 5:00 p.m.

Job Overview: As a Behavioral Health Care Manager, you will be a vital part of our collaborative care team, working alongside medical providers, psychiatric consultants, and a dedicated primary care team. You will support and coordinate the mental and physical health care of patients on your caseload, ensuring they receive the holistic care they need.

Key Responsibilities:

* Coordinate care for patients, working closely with their medical providers and other mental health professionals.

* Screen and assess patients for mental health and substance abuse disorders, and facilitate their engagement in treatment.

* Provide education on mental health and substance abuse disorders and treatment options.

* Track treatment progress and monitor for clinical symptoms, side effects, or complications.

* Support medication management and provide brief behavioral interventions using evidence-based techniques.

* Facilitate referrals to psychosocial treatments and external services as needed.

* Participate in regular caseload consultations and document patient progress in electronic health records (EHR).

Qualifications:

* Licensed Clinical Social Worker (LCSW) with 4 years of clinical experience, or Licensed Psychologist (PsyD/PhD) with 2 years of clinical experience.

* Experience with brief psychosocial interventions and working in primary care or community health settings.

* Strong collaboration and communication skills, with an ability to engage effectively with patients and care teams.

* Knowledge of evidence-based treatments, trauma-informed care, and psychopharmacology for common mental health disorders.
