JOB SUMMARY: Coordinates, evaluates and monitors an efficient and effective delivery of New Choices intensive outpatient and outpatient services consistent with best practices arid in accordance with agency funding source and licensing standards and expectations.

ESSENTIAL FUNCTIONS:

Program and Staff Development:

  • Supervises staff, monitoring and assessing performance of group and individual counseling and case management functions
  • Prepares annual evaluations and develops with staff, plans for improved performance
  • Plans, structures, facilitates and maintains group focus on recovery and skill acquisition
  • Establishes and maintains communication with referral sources and other agencies and treatment providers
  • Monitors staff productivity standard achievement
  • Monitors budget, manage budget, and participate in budget development
  • Monitors staff utilization of Perform Care electronic health record and Catholic Charities electronic health record for service delivery documentation and assure that the documentation meets state regulatory compliance

Quality Improvement:

  • Monitors the maintenance of accurate and up to date case records consistent with agency and funding source requirements
  • Utilizes supervision and training as an opportunity for enhancing professional competence
  • Participates in Quality Improvement activities, evaluating program effectiveness and outcomes
  • Prepares reports such as rosters, stats, and treatment recommendation letters to outside agencies

Direct Services:

  • Facilitates group when needed/has small case load when needed
  • Conducts and assures all staff complete thorough initial assessments utilizing EHR systems
  • Supports clients' strengths and resources, affirming reality testing, contact with experience, judgment and connection with support systems including family, friends and community resources
  • Involves and engages family and significant others in the recovery process and assists them in understanding the illness, coping with it and managing its effects
  • Consults with other service providers and referring entities and documents their participation in the recovery process
  • Maintains established program productivity requirements
  • Determines the appropriate level of care according to ASAM Criteria
  • Assesses clients using the ASI or other standardized clinical interview tool, and diagnosing clients for substance use disorder using the DSM-5
  • Participates as a member of a multidisciplinary team for assigned clients
  • Develops a client discharge/transfer plan to ensure movement to the appropriate levels of care

All regulations as follows:

  • Assessing the counseling needs of the clients;
  • Assessing clients using the ASI or other standardized clinical interview tool, and diagnosing clients for substance use disorder using the DSM-5;
  • Determining the appropriate level of care according to ASAM Criteria;
  • Obtaining previous records that are relevant to the current treatment episode;
  • Unless clinically contraindicated, collaborating with the client to develop a written treatment plan that includes goals and measurable objectives, and is client centered, recovery oriented; i.The director of substance abuse counseling shall develop, monitor and provide the client and treatment team a written schedule to update each client treatment plan(s) in order to ensure clients' needs are met;
  • Providing the substance abuse counseling services specified in the client treatment plan;
  • Reviewing clients throughout the treatment episode according to the ASAM Criteria, to determine the need for continued services, transfer, or discharge/transfer;
  • Reviewing and, where necessary, revising the client treatment plan to address ongoing problems;
  • Developing a client discharge/transfer plan to ensure movement to the appropriate levels of care;
  • Contact, case consultation if necessary, and coordination with referral sources, (for example, mental health treatment providers, criminal justice agencies, schools, employers, DCPP); N.J.A.C. 10:161B-1.9
  • Participating as a member of a multidisciplinary team for assigned clients;
  • Providing active case consultation; and
  • Documenting all counseling and education services, assessments, reassessments, referrals and follow-up in the client's clinical record, and providing appropriate signatures and dating of such entries, including those made in electronic records.
