Refugee Resettlement assists in helping create a home-away-from-home, we assist government-approved refugees in becoming self-sufficient through cultural orientation, job development, family reunification and resettlement.
Join the Refugee Resettlement team as the Medical Case Manager. The Case Manager I (Medical) is responsible for enrolling eligible refugee clients in Medical Case Management Program, performing thorough assessment, providing quality services and documenting these services in case notes. Will involve transporting clients to medical appointments, speaking with doctors and other medical professionals, and assisting Medical Case Management Coordinator in providing health literacy to refugee clients.
The Expertise We Are Looking For

  • High School Diploma or GED
  • One (1) year experience in an administrative setting or within refugee populations
  • Valid driver's license

  • You are able to demonstrate organizational and planning skills.
  • You are able to demonstrate success in dealing with a various type of people.
  • You are able to successfully document case notes.
  • You are able to provide group and individual orientations for clients.
  • You are able to handle confidential client information.
  • You have advanced knowledge and experience with MS Office Suite (Word, Excel or Access).
  • You have strong verbal and written communication skills.
What you Deliver
  • You assist Medical Case Management Supervisor in assessing potential clients for eligibility, providing intake and individualized service plans. Provide or arrange for interpreters when necessary.
  • You provide quality services to clients. Address identified needs through medical case management services, advocate for clients with clients' doctors and medical health professionals and facilitate communication between refugee clients and client's doctor and/or healthcare worker. Maintain clients' files and document any progress or change.
  • You perform other duties as assigned by Medical Case Management Supervisor such as providing health literacy workshops to refugee clients, providing training to interested medical professionals, and outreaching to and collaborating with to community stakeholders in efforts to provide more holistic services.
  • You perform other duties as assigned by the Supervisor, attending meetings with other service providers, participating in staff retreats, United Way tours, agency fairs and speaking engagements, assisting with special organizational projects/events.
