As the Family Case Manager, you will play a pivotal role in individuals' lives, making a significant impact through:
- Completes timely and accurate documentation in the Case Record, including but not limited to: referral intake packets and related intake information, referrals and related correspondences, entitlement documentation, and other areas as assigned
- Maintains notes in the case record to document progress toward, and/or achievement of milestones and performance targets as developed in the ISP
- Organizes and facilitates support groups and social and recreational activities, including but not limited to, issues relating to employment/vocational activities, daily living skills, treatment, housing permanency concerns, and entitlement eligibility
- Performs the intake and orientation of new referrals to the program during periods of high activity within the Center for Community Collaboration
- Cooperates in an ongoing supervisory process with the DSS, including but not limited to, attendance at regularly scheduled case management team meetings

Family Case ManagerSchedule: Monday-Friday: 8-4 or 9-5
Qualifications:
- Bachelor's Degree in a related field
- 1-3 years of experience
- Experience working with a diverse population
