Catholic Charities of the Diocese of Albany is Hiring a TBI Service Coordinator Near Colonie, NY

We are currently hiring for a TBI Service Coordinator, to work within our Catholic Charities Disabilities Services agency. As a TBI Service Coordinator, you would be responsible for:

  • Providing service coordination to individuals and families residing in the community at large
  • Maintaining current knowledge base of DOH policies, procedures and regulations related to service coordination
  • In general, the services entail helping individuals and families access appropriate programs, benefits and other opportunities that improve their quality of life

TBI Service Coordinator Minimum Salary: $20.51/hour

TBI Service Coordinator Maximum Salary: $23.07/hour

TBI Service Coordinator Schedule: M-F 8:30am-5:00pm; Combination of in-office and remote work

Qualifications:

  • Valid NYS Driver’s License
  • Experience is required. You must:
    1. (A) Be an individual with a Master’s degree/PHD in Social Work, Psychology, or related field. Or have one of the listed professional licenses:

LMSW, LCSW, Licensed Physical Therapist, Registered Nurse, Certified Teacher of Students with Disabilities, Certified Rehabilitation Counselor, Licensed Speech Pathologist, or Licensed Occupational Therapist

With one (1) year experience providing Service Coordination and information, linkages and referrals regarding community-based services for individuals with disabilities and/or Traumatic Brain Injury; OR 

  1. (B) Be an individual with a Bachelor’s degree and three (1) years’ experience providing Service Coordination and information, linkages and referrals regarding community-based services for individuals with disabilities and/or Traumatic Brain Injury; OR 
  2. (C) Be an individual with an Associate’s degree and five (2) years’ experience providing Service Coordination and information, linkages and referrals regarding community-based services for individuals with disabilities and/or Traumatic Brain Injury
