The Intake Service Coordinator, of Birth to Three Regional Administrative Unit, as a member of Catholic Charities West Virginia, is committed to advancing the mission of Jesus Christ, consistent with the core purpose and Gospel values enunciated in the Catholic Charities Code of Ethics.
The Intake Service Coordinator for the Birth to Three Regional Administrative Unit is responsible for accurately assessing, implementing, and updating a family’s need for Birth to Three services based on eligibility requirements.
Essential Duties
  • Assist clients with locating service coordinators.
  • Meet with families to determine eligibility by completing home visits.
  • Complete intakes and application process with clients.
  • Coordinate or participate in Child Find Activities, health fairs, etc.
  • Continually monitor and verify clients need for services.
  • Update records as needed, keep accurate documentation of activities.
  • Complete monthly reports reflecting the activity in current caseload.
  • Act as liaison between clients and service coordinators and direct service providers.
  • Work within time frames set by OMCFH & DHHR for new applications, development of IFSP’s, connection of families to service coordinators.
  • Makes contact with families after referral – by phone/letter/home visit.
  • Completing face to face interview with family/caretaker to identify family’s concerns.
  • Assist the family to identify practitioners to participate in evaluation and assessment activities related to their concerns.
  • If appropriate, completing activities to determine the family’s role in the evaluation and/or assessment process.
  • If appropriate, identifying potential natural environments and daily routines which evaluation and assessment activities should occur.
  • Confirming availability for evaluations with family selected practitioners.
  • Communication with all members of the child’s team throughout and up to the completion of the Individual Family Service Plan – IFSP.
  • Reviewing existing documentation including medical records, previous developmental assessments, screening results and sharing with other members of the child’s evaluation/assessment team.
  • Assuring that all members of the evaluation team have copies of the Intake Form and other relevant information prior to evaluation or assessment.
  • Facilitation and manual completion required paperwork for initial meeting with family and all team meetings up to and including the Eligibility and IFSP.
  • Adherence to all West Virginia Birth to Three time lines as they relate to eligibility determination.
  • Uploading information to the WV Birth to Three online system.
  • Assisting the evaluation team to provide the family with an eligibility statement that includes a review of pertinent records, reports the child’s status in five developmental domains and the status of the child’s eligibility, including categories in which the child is eligible.
  • Coordinating additional assessment activity if needed in order to proceed to development of the Initial Family Service Plan (IFSP) including assisting families to select practitioners.
  • Coordinating any and all transition procedures for the child and their family including a transition plan and resources according to state and federal guidelines.
Additional Duties
  • Collaborate with OMCFH & DHHR monitoring personnel.
  • Offer technical assistance to clients and providers.
  • Provide program information to clients and providers.
  • Records complaints lodged against clients and/or providers and refers them to the appropriate supervisor.
  • Record contacts with clients, service coordinators, and direct service providers.
  • Maintain confidentiality at all times.
  • Maintain accurate records of Birth to Three clients.
  • Travel as required to provide for client needs.
  • Meet with Social Services and public agencies to foster awareness and develop a working relationship.
  • Attends continuing education seminars to update credentials as applicable.
  • Assume additional duties as directed by the Director of the Regional Administrative Unit.
  • Performs clerical duties as needed (typing, filing, composing letters, etc.)
Supervisory ResponsibilitiesNo supervisory responsibilities.
Qualifications RequirementsTo perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required.
  • Bachelor’s degree in Human Services Field such as social work, early childhood, special education, elementary education, nursing, psychology, and counseling.
  • Preferred WV Social Workers License or Bachelor’s Degree in a field approved for a West Virginia Temporary Social Work License.
  • Ability to work with persons from varied socio-economic backgrounds.
  • Prior experience and willingness to work with people of cultural, ethnic, and racial diversity.
  • Must be eligible for certification established by the West Virginia Birth to Three Program.
  • Ability to maintain confidentiality.
  • Good written and oral communication skills.
  • Ability to work collaboratively in a team environment.
  • Pass background check.
