General Position SummaryThe Personal Agent provides trauma-informed case management services to persons with intellectual and/or developmental disabilities, using person-centered practices to assist individuals in identifying support needs and life goals then providing guidance and support with accessing services to achieve those goals and meet those needs.
The ISN Personal Agent Float position is required to work varying schedules, workloads and locations based on program coverage needs. This position will be responsiblefor keeping current on various program guidelines and processes to ensure they can quickly step in to support coverage. This position will usually not carry an independent caseload, with the expectation that they are available to assist with coverage needs for personal agents and assessment coordinators in any ISN location when the need arises as well as with program projects as identified by the program director when limited caseload coverage is needed.Benefits We Provide
Two years Personal Agent/Case management experience in in-home services in Oregon.
Reliable transportation, valid driver’s license with a driving record that permits coverage under the agency’s
corporate auto liability policy, and personal automobile liability insurance coverage.
Preferred Qualifications Previous experience in Person Centered Planning and Self-Determination .
