• One (1) year acute care experience or
• One (1) year home healthcare experience
• Wound care certification a plus!

• Be a master scheduler and great multitasker when it comes to planning, coordinating, providing, and documenting care for a geographically assigned caseload of patients from day of admission through day of discharge or death.
• Performs admission assessment and evaluation of patient within 48 hours of referral or return to his/her residence.
• Demonstrates knowledge of medications and their correct administration based on the patient’s clinical presentation.
• Be an advocate for our patients by initiating case conferences to discuss multidisciplinary team responsibilities, patient progress, plans for continued care, new problems, etc.
