We are in need of several home health nurses, who can provide care to patients within Central Phoenix, South Phoenix, and the West Side.

Celebrity Home Health has a very small turnover rate for Registered Nurses, as it is our practice to:

• Provide Continuity of Care; thus the nurse is never required to share a case. Once the patient is admitted the nurse who admitted the patient provides all the visits needed for the patient.
• Provide Excellence in Care; thus the nurse is not obligated to accept patients. Our nurses only accept the patients they are comfortable providing care for.
• Provide the time needed for the best patient outcome; thus the nurse has the power to accept or refuse cases in order to maintain a healthy work/life balance, which makes for better patient outcomes.
• Minimize paperwork; thus allowing more of your time to caring for the patient.
• Respect your schedule; by respecting our nurses schedule it enables the nurse to better accommodate the patients needs.
• Recognize that it is the Nurse who provides care; along with, ensuring better patient outcomes; thus office staff backs up the clinical staff by, scheduling the lab when tests are ordered, ordering the wound care supplies needed, or DME when required, and sitting on hold when the doctor needs to be reached. Allowing the nurse to do what is needed in the field; provide care to patients that leads to better patient outcomes.
• We do not schedule patients; We leave it up to the nurse to schedule the visits required as long as it follows the MD Order and the Medicare week in order to accommodate your work/life balance.
• Your patient caseload will consist of as many or few patient visits that work for your schedule. Whether you chose a full time schedule of visits or a part time schedule, no patients will ever be forced upon you.
• We do not require you to accept patients out of your specific service area.
• Mileage is paid according to IRS specifications
• Eligible for 401k after one year; with a company match

Join our nursing team at Celebrity Home Health and Hospice. We are seeking skilled Registered Nurses able to work independently, who will be responsible for improving a patient's quality of life by planning, directing, and providing Skilled Health Care services. These licensed nursing professionals will provide direct nursing care to treat patients under a physician's plan of care and in accordance with the policies and procedures of the agency. The Nurses goals are to rehabilitate and educate the patient to regain and retain independence in their home setting. The provided care plan will include all clinical care, symptom control and identify other physical, psychological, social and spiritual needs for assigned patients.
