We are currently seeking a highly organized and detail-oriented Clinical Case Coordinator for our Intensive Services Unit. Rapidly after release into the program, the Clinical Case Coordinator conducts Supervision Introduction/Orientation meetings with new participants to review supervision conditions that will be required under the supervision plan. The Clinical Case Coordinator assists participants on their case load to comply with supervision appointments and court appearances during the pendency of their legal case. In this role, you will conduct ongoing service need assessments with participants and develop collaborative service plans addressing identified needs and goals. You will work alongside a Peer Specialist by assisting program participants together and focusing on delivering proactive field-based direct services and limited coverage at court.
You will ensure participants receive services that are strength-based, gender-specific, and culturally sensitive while adhering to the core principles of risk, need and responsivity to reduce recidivism, increase engagement in community treatment services and support participants to achieve their self-identified goals for treatment, recovery, rehabilitation, and crime-free community integration. This position is based in the program’s respective borough office and field work in the larger NYC community as needed to assist participants. The ideal candidate for this position will possess excellent organizational skills, effective communication abilities, and the capacity to prioritize tasks efficiently.

Location: Harlem
Salary: The salary range for this role is $62,500 - $67,500 per year.
Work Schedule: Monday-Friday; 1pm-9pm
Remote Flexibility Status: Flexible. This job requires work in our offices and or/ in the community; however, the role allows for some remote work throughout the week, as directed by the supervisor.
