Position Summary:

Job Description

Provide family focused case management to community adults, adolescents, and families to enhance coordination of care and the prevention, treatment, and recovery services available for individuals with or at risk for SUD/BH in Schuylkill County, PA.

• Develops & Maintains Service Plan in conjunction with consumer and other collateral agencies that identifies needs, desired outcomes, and resources needed.
• Linking: Case Managers will assist the consumers in locating and obtaining services specified in the treatment or service plan, including arranging for the consumer to be established with the appropriate service provider.

Essential Functions

• Manage all personnel matters including hiring, supervision, training and evaluation of assigned staff.
• Maintain good working relationships across departments and programs internally as well as with governmental agencies and community organizations.
• Provide family focused case management to eligible individuals/families in the community.
• Assist individual/families with service planning.
• Act as the individual/family’s single point of contact for multiple health and social services systems linkages in the identified SDOH.
• Link individuals/families to community-based services and support.
• Monitor the individual/family’s participation in services and provide support to encourage participation.
• Maintain quality standards.
• Other duties as assigned to support the mission of the agency.

Bachelor’s degree in chemical dependency, sociology, social welfare, psychology, nursing or relate field or a bachelor’s degree which includes successful completion of 12 college credits in sociology, social welfare, psychology, criminal justice, chemical dependency, or related social science. 1 year experience in the mental health field and/or 1 year experience in D&A field.

Job Type: Full-time

Pay: $22.54 per hour

Expected hours: 37.5 per week
