Center for Mental Health is Hiring a VA Care Coordinator - CCBHC Near Great Falls, MT

 

VA Care Coordinator

Certified Community Behavior Health Clinic

Full-Time, Non-Exempt

Any Location

Job Duties: 

  1. Provides the VA with a set contact person regarding referrals.
  2. Completes VA authorizations and tracks clients.
  3. Develops partnerships with the Department of Veterans Affairs and establishes care coordination agreements with these facilities.
  4. Fulfills CCBHC requirements for Criteria 4K: Intensive, Community-Based Mental Health Care for Members of the Armed Forces and Veterans.
  5. Principle Behavioral Health Provider for veterans/service members.
  6. Administers vital signs, review medical histories with clients, conduct UA and Saliva testing, triage phone calls, and client scheduling as needed.
  7. Ensure client`s care is coordinated between client and all healthcare providers, provides continuous collaboration with local agencies to establishment continuity of care for the clients. 
  8. Ensures clients` access to needed services during transitions from different levels of care. 
  9. Provides education, reinforcement, and frequent communication with clients about their physical and behavioral health status. 
  10. Enters CCBHC data specific information into electronic health records. 
  11. Provides encouragement and support to clients to assist them in solving difficulties they may have managing and improving their health. 
  12. Grant funded position ending September 29, 2027.

Minimum Qualifications: 

  • Minimum qualifications are a bachelor’s degree in a human services field and considerable knowledge of the principles, methods, and skills needed to effectively preform case management duties. 
  • Other education and experience may be considered. 
  • Must have excellent computer skills such as Excel, Internet, Teams, Outlook, and Word. Familiar with medical diagnoses and medications is preferred.
  • Travel required. Must have vehicle, valid driver’s license, and may transport clients.
