CENTER FOR URBAN COMMUNITY SVC is Hiring a Medication Case Manager (Times Square) - S1 Near New York, NY

The Center for Urban Community Services, (CUCS), is an acclaimed human services organization and one of the nation's largest providers of housing and social services. CUCS is a leader in implementing new practices, sharing knowledge and assisting in shaping local, state and national strategies, so that persons who are homeless, low-income, living with mental illness, or have other special needs can live successfully in the community. 

The Medication Case Manager is responsible for working alongside the site’s LPN and onsite medical providers to monitor medications for the shelter, transitional and safe haven residents. Assistance with Self Administration of Medication is a fundamental part of our program model. The Medication Case Manager will work alongside all staff to help our clients live full and satisfying lives in the community; helping their team and program to function well and meet their contractual obligations; and striving, as appropriate to their role, to fulfill the CUCS programs’ core purpose, core values and vision statements. 

Responsibilities: 

The primary responsibilities of the Medication Case Manager are, but not limited to: 

  • Treat all clients in a respectful and dignifying manner.
  • Monitor clients self-administering their medication, seek out clients within the facility, provide counseling to encourage medication compliance, and assist with ordering and receiving of medications.
  • Responsible for keeping the medication room and supporting documentation organized, up to date, and in compliance with agency policies.
  • Responsible for maintaining internal Client Management System (CMS), other internal databases/systems and external funder systems
  • Assist clients with the self-administration of medication as necessary following agency policies and procedures, proactively identifying and resolving or reporting problems, helping people move towards greater independence managing their medication.
  • Complete progress notes according to agency and funding source guidelines regarding timeliness,
  • Complete all other service documentation according to agency and funding source guidelines regarding timeliness,
  • Achieve on time completion of service documentation without ongoing reminders from supervisor,
  • Provide documentation that is accurate, relevant, concise, appropriate to the setting, and supportive of quality services.
  • Engage clients in a working relationship that is appropriate to their service needs.
  • Know each client well enough to carry out the job responsibilities (Goals, concerns, interest, diagnosis, medical regiments, risk factors, psychosocial history, collateral contacts)
  • Participate actively in weekly supervision, and work proactively to create an effective relationship with supervisor,
  • Take initiative in setting supervisory agenda, and provide supervisor with appropriate level of information regarding work activities,
  • Participate in trainings as needed, required, or assigned; and integrate material from trainings into own work,
  • Adjust to new assignments and changing workload; adjust priorities as required; maintain composure and productivity under pressure and in challenging circumstances,
  • Complete assignments within established time frames,
