We are currently accepting applications for compassionate, motivated EXPERIENED REGISTERED NURSES for our CHHA division. We strive to provide QUALITY services to our patients.
Duties And Responsibilities
Duties And Responsibilities
- The Field RN (Registered Nurse) performs home visits, assessments and follow up visits to patients.
- The RN advocates for quality patient care, collaborates with Physicians and other providers to maintain a safe and effective home environment for patients.
- The Registered Nurse works closely with patients, families and interdisciplinary team to ensure quality care is provided at home.
- Must be a NYS Registered Nurse
- Experience working for a CHHA or Prior Hospital Med/Surg experience
- Ability to visit patients in a home environment
- Willingness to collaborate with interdisciplinary team as needed
- Excellent organizational skills required
- Knowledge of ICD 10, Medicare/Medicaid and OASIS guidelines a plus
- Ability to drive and have access to reliable automobile preferred
- Bilingual Chinese, (Cantonese/Mandarin) Russian, or Spanish a plus
